Storing tirzepatide: the 21-day, 30-day and 4-dose limits

Tirzepatide's storage rules depend on which container you have, and the two containers that look most alike, a single-dose vial and a multi-dose vial, have different limits. Everything here is from section 16.2 of the Zepbound label; the Mounjaro label carries the same limits for the same presentations.

Rules for every presentation

Do not freeze. Do not use tirzepatide if it has been frozen. Protect it from heat and light, and store it in the original carton. Those four sentences come before any of the container-specific limits.

Single-dose pen and single-dose vial

Store in a refrigerator at 2°C to 8°C (36°F to 46°F). If needed, each pen or vial can be kept unrefrigerated at temperatures not exceeding 30°C (86°F) for up to a total of 21 days. After a total of 21 days at room temperature, discard it.

The word "total" is doing work. The 21 days are cumulative across the life of that pen or vial. A pen that spends four days in a suitcase and then goes back in the fridge has 17 room-temperature days left, not 21. Since single-dose products contain one injection each, the practical question is usually whether a four-pack that travelled with you for a week is still usable: it is, for the remaining three doses, as long as the temperature limit was respected.

Multi-dose vial and single-patient-use KwikPen

These contain four weekly doses in 2.4 mL and have two sets of limits.

Unopened: refrigerate at 2°C to 8°C and use until the expiry date on the label. If stored unopened at room temperature (up to 30°C), discard after 30 days.

Opened, meaning after the first dose has been taken: keep it in the original carton, either in the refrigerator or at room temperature up to 30°C, and throw it away after a total of 30 days at room temperature, 30 days after first use, or after taking four weekly doses, whichever comes first, even if there is medicine left.

For a weekly schedule the four-dose limit and the 30-day limit arrive within a couple of days of each other, so the KwikPen or vial is normally finished before either bites. A missed dose changes that. Four doses over five weeks is 29 to 35 days from first use; if the fourth dose falls past day 30 the label says discard, and the dose is lost.

Compounded vials

None of the above applies to a compounded product. Compounded tirzepatide is not FDA approved, is not interchangeable with Zepbound or Mounjaro, and its stability has not been established in the studies behind the brand label. Its beyond-use date, storage temperature and handling come from the compounding pharmacy's label and nowhere else. If the pharmacy label says 28 days after opening, that is the rule. If it does not state a limit, ask before the first draw, not after.

Heat, travel and mistakes

The label sets 30°C (86°F) as the ceiling for unrefrigerated storage. A car interior on a summer afternoon exceeds that quickly, and the label gives no allowance for brief excursions above it. It also gives no way to test whether an overheated or frozen dose is still active; the instruction is to discard a frozen one and not to exceed the temperature limit at all. For travel, a small insulated pouch that keeps the carton under 30°C and away from light satisfies the label for single-dose products for up to 21 cumulative days.
