Zepbound self-pay prices: every Lilly price with its date

Lilly has published four sets of Zepbound cash prices since approval. This page lists each one with its date and source, because a price without a date is the most common error on GLP-1 cost pages, and then turns the current prices into the numbers that matter: per injection, per milligram, per year, and for the first year of escalation.

The price history

Each row is what Lilly said on that date. The June 2025 release also stated that 12.5 and 15 mg vials could be prescribed from July 7, 2025, with shipments from early August 2025; before then, vials stopped at 10 mg. The December 2025 release framed the cut as building on efforts to lower out-of-pocket costs; the previous range had been $349 to $499.

What the current prices work out to

At $449 for four 15 mg doses: $112.25 per injection, $7.48 per milligram, and $5,853 a year (a year is 13.04 fills of 28 days, not 12). At $449 for four 7.5 mg doses: same per injection, $14.97 per milligram. At $299 for four 2.5 mg doses: $74.75 per injection, $29.90 per milligram.

The first 52 weeks, escalating at the label minimum of four weeks per step to 15 mg, is one fill each at 2.5 mg ($299), 5 mg ($399), 7.5, 10 and 12.5 mg ($449 each) and then eight fills at 15 mg ($449 each): $5,637. To 10 mg it is $299 + $399 + $449 + ten fills at $449, also $5,637, because every strength from 7.5 mg up costs the same. To 5 mg it is $299 + twelve fills at $399, $5,087. What the prices do not include

The prescription. LillyDirect connects to telehealth providers, and independent prescribers charge their own visit fees. Lab work, if your prescriber wants it. Syringes and sharps disposal for vials; the single-dose vial is drawn with a syringe you supply. Shipping where charged. And the 45-day refill condition: the programme's discounted price for 7.5 mg and higher requires refilling within 45 days of the previous delivery, so a lapse costs the difference between $449 and the standard price for that fill.

Compounded prices

This page tracks Lilly's prices for FDA-approved Zepbound. Compounded tirzepatide is priced by each pharmacy and telehealth programme, is not FDA approved, and is not interchangeable with Zepbound or Mounjaro; a lower monthly figure is a different product, not a discount on the same one.
