per injection = price per 28 days / 4 per mg = per injection / weekly dose (mg) per year = price per 28 days x 365 / 28 (13.04 fills) first 52 weeks = sum over 13 fills of price at the dose in effect (label minimum: one fill per 2.5 mg step)
Example: LillyDirect vials at 10 mg, $449 per 28 days (December 1, 2025 price). That is $112.25 per injection, $11.23 per mg, and $5,853 a year. Escalating from 2.5 mg at the label minimum, the first year is $299 + $399 + $449 + 10 x $449 = $5,637.
Assumptions, checked September 4, 2026
- A 28-day supply is four weekly injections. Lilly's single-dose vials come as 4-packs and the KwikPen holds 4 doses, so one fill equals 28 days.
- Prices are the ones Lilly published on the dates shown. They are cash prices for people paying out of pocket with a valid prescription; they exclude the prescriber visit, shipping where charged, and any lab work.
- LillyDirect's program terms tie the 7.5 mg and higher price to refilling within 45 days of the previous delivery. Miss the window and Lilly's standard self-pay price applies.
- The savings-card option models the advertised floor only. Actual out-of-pocket depends on your plan's copay and the card's $100 monthly and $1,300 annual limits. Government coverage is excluded.
- The list price is Lilly's stated WAC on the approval date and is included as a reference point. It has changed since and pharmacies do not sell at WAC.
- Compounded prices are not modelled because they vary by pharmacy; enter a quote. Compounded tirzepatide is not FDA approved and is not interchangeable with Zepbound or Mounjaro.
